fix(cgpt-actions): add sys.path fix for ActionMonitor import and fix JSON serialization
...
- Insert agent-os path into sys.path so orchestrator module is found
- Fix JSON serialization of ActionFailureEvent by converting dataclass to dict in the endpoint
- Add __init__.py for orchestrator package
This is a bugfix on the V1R2C2 feature already merged into main.
