AIOS Governed Continuation 5e4d87dcf6 fix(task_plane): caller_scan default conservative UNKNOWN (spec-aligned, QA note) ...

- allow_unknown_lower default True->False: opaque dynamic import now returns
  UNKNOWN by default, never downgraded to orphan unless caller opts in (V1 spec
  red line). Update docstring; test updated + new allow_unknown_lower flag case.
- QA review note resolution. Tests: test_caller_scan 8/8 green.
