feat(sr01): add consistency sentinel, safe-mode gate, and test suite

- Integrate gitea_branch_consistency_sentinel.py (bare repo refs ↔ Gitea SQLite ↔ API)
- Integrate autonomous_safe_mode.py for controller mutation gate
- Update autonomous_controller.py with safe-mode checks before writes
- Add tests/test_autonomous_controller.py (11 tests covering h_branch_sync, h_stuck, apply_approved_patches, diagnostics)
